1. To register, please download the form here.

2. Deposit registration fee of P3,000 to UA&P – BPI ACCOUNT NO. 0201-041473.

3. Accomplish registration form. Do not forget to include your payment details.

4. Send an electronic copy of the completed registration form and deposit slip to history@uap.asia or fax to 637-0912 loc. 369.

5. Present your deposit slip on April 27.
